Acceptance of These Terms

These Terms & Conditions (“Terms”) govern your use of DeliciaChiro.com (the “Website”), operated by Dr. Delicia Paredes Chiropractic (“we,” “us,” or “our”). By using the Website or submitting any form on it, you agree to these Terms and to our Privacy Policy. If you do not agree, please do not use the Website.

Website Content Is Not Medical Advice

Information on the Website is provided for general educational purposes only. It is not medical advice and is not a substitute for an in-person examination, diagnosis, or treatment by a qualified health care provider. Do not disregard professional advice or delay seeking care because of something you read on the Website.

Using the Website, reading its content, or submitting a form does not create a doctor–patient relationship. A doctor–patient relationship begins only after you are evaluated in our office and accepted as a patient.

Medical Emergencies

Online Forms and Appointment Requests

• Appointment requests are not confirmed appointments. An appointment is scheduled only after our office contacts you to confirm a date and time.
• Accurate information. You agree that the information you submit, including on the New Patient Intake Form, is complete and accurate to the best of your knowledge, and that you are the person named or are authorized to submit it (for example, as the parent or legal guardian of a minor).
• Electronic signatures. When you sign a form on the Website by drawing your signature and checking the related boxes, you agree that your electronic signature has the same legal effect as a handwritten signature.
• Review by our office. We may contact you to clarify or complete information, and we may ask you to review and re-sign forms at your first visit.

Consent to Calls, Text Messages, and Email

When you check the consent box on any form on the Website, you agree that we may contact you by phone call, text message (SMS), and email, using the phone number and email address you provided, about your inquiry, appointment requests, appointment reminders and scheduling, your care, billing, and office notices.

• Message frequency varies based on your requests and appointments.
• Message and data rates may apply, depending on your mobile carrier and plan.
• You can opt out of text messages at any time by replying STOP. Reply HELP for help, or call us at (562) 415-8209.
• You can withdraw your consent to calls, texts, or email at any time by telling us by phone or in person. We may still contact you as required to complete a request you made or as permitted by law.
• Mobile carriers are not liable for delayed or undelivered messages.
• We do not sell your phone number or share it with third parties for their marketing purposes.

If you prefer not to give this consent, you can call our office at (562) 415-8209 instead of using the online forms.

Standard email and text messaging are not fully secure. Please avoid sending detailed health information by email or text unless we have arranged a secure method with you.

Office Policies

Our office policies are presented on the New Patient Intake Form and apply to patients of the practice, including:

• Cancellations: please give at least 24 hours’ notice. A $25 fee may apply to missed appointments or late cancellations.
• Payment: deductibles, co-payments, co-insurance, and other fees are due at the time of service unless other arrangements have been agreed upon in advance.
• Returned checks: a $25 fee applies to all returned checks.
• Insurance: information about insurance on the Website is general. Coverage depends on your specific plan, and verifying benefits does not guarantee payment by your insurer.

Intellectual Property

The Website and its content, including text, graphics, logos, and images, are owned by or licensed to Dr. Delicia Paredes Chiropractic and are protected by copyright and other laws. You may view and print pages for your personal, non-commercial use. You may not copy, republish, or distribute Website content for any other purpose without our written permission.

Acceptable Use

You agree not to:

• submit false information or submit forms on behalf of another person without authorization;
• send spam, automated submissions, or malicious code through the Website;

Governing Law

These Terms are governed by the laws of the State of California, without regard to its conflict-of-law rules. Any dispute relating to the Website will be brought in the state or federal courts located in Los Angeles County, California.

Changes to These Terms

We may update these Terms from time to time. The “Last updated” date at the top of this page shows when they were last revised. Your continued use of the Website after changes are posted means you accept the updated Terms.
